Submit a correction

Placeholder page — this stays a stub, not a working form, until thecorrections table's unconditional public-INSERT RLS policy is closed (tracked in docs/stage-0-decisions.mdas a pre-launch security item). Building the form against that policy as it stands would let anyone write unlimited rows to the database.
